Active Surveillance in Localized Prostate Cancer: When and How

|
The natural history of prostate cancer is heterogeneous and it is not completely understood. Prostate specific antigen (PSA) screening has led to an increased incidence of detecting low risk, localized, clinically insignificant tumors. Active surveillance may be a treatment option for patients with low risk, localized prostate cancer. The selection criteria for active surveillance are being defined. The patients with low risk, localized tumor characteristics are closely monitored by assessing the PSA level and PSA kinetics. The choice between radical treatment and active surveillance is based on any evidence of disease progression, and this is defined in terms of the PSA doubling time and "upgrading" on a repeat biopsy. In the future, active surveillance may accelerate the progress towards a new treatment paradigm for low risk localized prostate cancer.
